renal associates financial policy

As a courtesy, all health insurance claims will be processed with your authorization. Please be prepared to provide your current health insurance card, photo identification, and updated demographic information at the time of
each visit.

Our clinic participates in the Medicare and Louisiana Medicaid programs, and is contracted with most insurance plans in the area. Direct payment is accepted from all insurers. Insurance carriers often require referrals or pre-
authorization of services. We are happy to assist you in obtaining required authorizations.

Insurance carriers, including Medicare, usually require a co-pay and/or coinsurance, and apply deductibles for medical services. This means that you are likely responsible for at least a portion of the cost of the service. We ask
that you familiarize yourself with your insurance policy to understand your payment responsibility. Payment is expected at the time of service for clinic appointments.

Full payment is expected within 60 days from the date of service.  We accept Cash, Checks, and major credit cards as forms of payment.

New patients receiving services from Renal Associates physicians who do not carry medical insurance should speak to our financial counselor to make special arrangements for payment of medical services.

We have staff available to discuss any billing issues with you or assist with insurance coverage questions during normal business hours.
